IBC Protocol Form
Please use the new PDF forms for biosafety submissions. You should fill them out and email them to Andrew Cockburn (acockbur@mail.wvu.edu). Each submission will require the basic protocol form, the additional personnel form if appropriate, and at least one form for the specific biohazard(s) you are planning to work with.
You may combine similar items into one form: for example, if you are working with non-hazardous genes in standard E. coli cloning vectors, only one rDNA form is needed. (Hazardous genes include coding sequences for toxin genes, oncogenes, large sections of eucaryotic viruses, and any other genes that are likely to code for dangerous products.) In this case, the work is exempt.
